Rhetorical Answer

San Franciscans, as well as Californians in general, are often maligned for their incomprehension of drier wit. And although answering rhetorical questions is more often the domain of Eastern liberal-arts academicians, the local author of this graffito felt a similar need to point out the obvious. We probably all agree that there are lots of things in hospitals that are bad for you: corrupt HMO doctors, drunk surgeons, rotting smells. It is unclear to which "bad thing" this author is referring, but nothing we could come up with was worse for you than having to watch ABC's Friday night lineup.
